Passport Rehabilitation Program

The goal of our Passport Program is to restore you to health. We understand that every person under our care has distinct requirements for therapy. Your individual rehabilitation is tailored to a series of realistic, short-term achievements based on getting you home. 
Our services are designed to give you the skills and abilities needed to live a healthy and fulfilling life. Whether you’re struggling with physical or post-operative challenges, our dedicated staff will cater to your unique needs and preferences, providing customized therapy in comfortable and finely appointed accommodations.

Often times individuals come to us on a planned basis following a pre-scheduled surgery. Make us part of your “journey” to a more fulfilling life by pre-booking your room and therapy services in advance of your surgery. We will handle all of the “paperwork” for you and your surgeon, by working with Medicare and your insurance carrier to give you one less thing to worry about. Long-Term Skilled Nursing Care

Alzheimer's/Dementia Care

Many NHCA centers offer specialized services to meet the needs of individuals with a wide variety of dementia-related disorders, including Alzheimer's disease. Many of those centers provide:

  • Wander Guard Security Systems
  • 24 Hour Skilled Nursing Care

Respite Care Program

Caring for someone in need of medical and emotional support requires tremendous energy. For those times when a caregiver needs some relief, whether for a few days or a couple of weeks, NHCA offers respite services. An individual normally cared for at home or similar setting can be admitted to a NHCA center for a short-term stay in order to provide the family caregivers a chance to recover from the stresses of daily care, take a vacation or focus on other personal needs. Services offered in conjunction with a respite stay include:
  • 24 Hour Skilled Nursing Care, as needed
  • Medication Management
  • Recreation Programs
  • Physical and/or Occupational Therapy, if indicated
  • Nutritional Needs Management
  • Family Support Counseling

Palliative Care & Hospice Services Coordination

Working in conjunction with certified Hospice care providers, NHCA offers a wide range of clinical and supportive services aimed at providing comfort and dignity to individuals with terminal illnesses. Services offered within NHCA's palliative care model include:
  • 24 Hour Skilled Nursing Care
  • Medication Management
  • Aromatherapy
  • Spiritual and Emotional Support
  • Recreational Programs, as appropriate
  • Family Counseling and Care Participation

Pulmonary Disease Management Program

Our Pulmonary Rehabilitation Programs are designed to maximize the functional abilities of persons with Pulmonary Disease.

Heart Failure Program

Our Heart Failure Program is designed to provide education, evaluate the many intricacies of heart failure including the underlying causes, medications and necessary lifestyle modifications. 
  • Advanced Practice Registered Nurse (APRN) on Staff
  • Adherence and Understanding of the American Heart Association Guidelines for Diagnosis/Management
  • Onsite Automatic External Defibrillators (AED) to address any emergency needs
  • Smoking Cessation Counseling

Ventilator Program

Our Passport Ventilator Care program is focused on taking an aggressive multi-disciplinary approach to ventilator rehabilitation. Our ventilator weaning program is focused on reducing or eliminating dependence on a ventilator for breathing.  Clinical expertise allows us to design an individualized respiratory health plan that incorporates physical, occupational and speech therapy. For those requiring chronic ventilator services our program is designed to provide optimum care and comfort. 
  • Four Levels of Care: Ventilator, Tracheostomy, Short-term and Long-term Pulmonary
  • Respiratory Therapist on Staff 24 Hours Per Day, 7 Days Per Week
  • Pulmonologist On Staff
  • “Out of Bed” Philosophy for Patients to be Out of Their Room for Meals, Therapy and Other Activities
  • Support Groups for Patients and Family Members, Led by Licensed Social Workers
  • Separate Unit on Campus with Therapy Gym, Dining Rooms, Wi-Fi and Lounge Areas
