For over 30 years, National Health Care Associates (National) and its affiliate centers have delivered quality post-acute care. Our growing network of short-term rehabilitation and long-term care centers is located throughout Connecticut, Maine, Massachusetts, New Hampshire, New York, and Vermont. Our Passport™ approach to better health combines compassionate care with clinical excellence to deliver innovative solutions across the care continuum, including managing patients post-discharge. 

According to the Centers for Medicare & Medicaid Services (CMS), less than 53% of patients successfully return to their home or a community-based service following a short stay in a SNF.

At National we don’t settle for 53%. We go further to make sure our guests return home safely and continue to recover with their own compassionate, skilled care team.

Post-Acute Rehabilitation

Our Passport approach to short-term care focuses on helping individuals transition home—or where they need to be—as fast as possible. For temporary post-acute care following a surgery, illness, or other medical condition, our Passport to Home process offers the best amenities, clinical care, trained and specialized staff, up to seven days a week therapy, transitional care management, and innovative technologies including remote patient monitoring.

Long-Term Care

National’s centers are uniquely designed to provide people who require care beyond a short-term stay with access to a long-term area dedicated to the highest level of quality care, customer service, and a nurturing environment for our guests and families. Each home offers around-the-clock care for those living with chronic illness or disabilities and combines companionship with dining services and daily recreation, which are vital components in the journey to better health. Additionally, we offer assisted living services at the following locations for individuals who may have issues with mobility or personal care but do not need 24-hour support.

Founder, Owner, President

Marvin J. Ostreicher

Marvin Ostreicher is the founder, owner, and president of National Health Care Associates. Under Marvin’s direction, National has expanded its footprint to include over 30 facilities in six states, with a workforce of 8,500 caregivers managing thousands of patients. By fully understanding the complete continuum of care, he has diversified National to include a home health care company, a therapy company, and an institutional pharmacy. Under Marvin’s leadership, National has become one of the largest and most respected long-term care providers in the northeastern United States.

Marvin holds a bachelor’s degree from Yeshiva University and worked in the healthcare industry for eleven years before founding National.

Chief Operating Officer

Ephram Ostreicher

Ephram Ostreicher, Director of Business Operations, leads the integration of post-acute services across the care continuum, including home health, hospice, health tech, and diagnostic services. Ephram has helped organizations achieve strategic business goals as a healthcare executive within the post-acute and long-term care industries. He was previously an Administrator and COO at NHCA, overseeing the daily operations of our facilities, and has been involved in various CMS value-based care initiatives since 2015. Ephram received his MBA from Johns Hopkins University with a Graduate Certificate in Seniors Housing and Care.

Chief Financial Officer

Thomas J. Gilmartin

Thomas J. Gilmartin, CFO, oversees and manages the financial operations for NHCA and our skilled nursing facilities. Thomas has extensive experience in healthcare finance in the long-term care and community service industries. He earned his BS in Accounting from Herbert H. Legman College and holds an MBA from Norwich University.

Chief Clinical Officer

Michelle Costa

Michelle Costa, Chief Clinical Officer, is a nursing executive with over 20 years of leadership and management experience. She has improved clinical outcomes through organizational change, and elevated nursing standards across departments and divisions while using evidence-based theory to successfully ensure achievement of clinical goals. Michelle’s clinical perspective defines National’s standards of care, while enhancing patient outcomes and experiences across our care continuum. Michelle received her Doctorate of Nursing Practice from Chamberlain University in 2022, and also holds a master’s degree in Health Care Administration and Management from Salve Regina University.

Chief Human Resource Officer

Santosh Chitalia

Santosh Chitalia, CHRO, oversees NHCA’s recruiting, retaining, and engagement of employees with a people-first approach that aligns with our core mission. Santosh previously worked as a healthcare attorney in private practice and joined NHCA as a compliance officer. He since took on an operational role to serve our facilities before advancing to become CHRO. Santosh holds a Juris Doctorate from New York Law School.
