National Health Care Associates and ProHEALTH Care New York Announce Collaboration
VALLEY STREAM, NY — National Health Care Associates (NHCA) and ProHEALTH Care New York (ProHEALTH) recently announced a collaboration aimed at coordinating care for Long Island’s seniors in need of short-term rehabilitation after a hospital stay or urgent care center visit.
The collaboration narrows the network of skilled nursing providers ProHEALTH works with in an effort to improve communication and better track outcomes and patient movement from hospital to home. Belair Nursing and Rehabilitation Center in North Bellmore, Huntington Hills Center for Health and Rehabilitation in Melville, and Sands Point Center for Health and Rehabilitation in Port Washington will work closely with ProHEALTH hospitalists and care managers.
“We are constantly working to find new ways to improve the quality of care for those we serve,” said Anthony Ardito, M.D., Chief Medical Officer for ProHEALTH Care New York. “When it came time to identify how we could best track our patients, particularly when they found themselves in one of Long Island’s more than 7,000 nursing home beds, National Health Care Associates quickly rose to the top as the group who was willing to work with us to try something new.”
ProHEALTH is a multi-specialty integrated healthcare organization that operates 30 urgent care centers throughout the greater Long Island area and provides physician services for many of the area’s acute care hospitals. The collaboration is focused around NHCA’s Passport Rehabilitation Program with an emphasis on cardiac, neurology, orthopedic and pulmonary conditions for short-stay patients.
